Severe Fracture-dislocation of the Lumbar Spine Caused by 2 Point Lap Belt in an Adult - A Case Report

Abstract
We present a case of significantly displaced lumbar fracture-dislocation with complete neurologic injury sustained by a 29 year man, involved in a motor vehicle collision, while wearing a 2 point lap belt. Radiological examination revealed a transversely oriented fracture that extended through the L1 vertebral body with complete anterolisthesis and spinal cord disruption. Neurologic examination showed paraplegia with a T9 sensory level. Eight days after these injuries, the patient underwent open reduction and fusion with bone graft and instrumentation. Six months later, the patient is undergoing rehabilitation and has no volitional motor control inferior to his umbilical level. This case demonstrates the potentiality of a 2 point lab belt injury producing a devasting Chance fracture in the adult patients.
